| Process Phase | Input | Output | Description |
| ------------------ | ----------------------------- | ---------------------------- | ------------------------------------------------------------------------------------------------------------------------------------------- |
| **Identification** | Desire & energy | List of known processes | This is the first step, and one you do on an ad hoc basis thereafter. Here you go from not having a list to work through, to having a list. |
| **Discovery** | SMEs & existing documentation | Process models | This is modeling the **current state** of the process(es) you're working toward improving. |
| **Analysis** | Process models | ID'd problems & [[Metrics]] | You identify & document problems or deficiencies with the existing process, and look for ways to measure it. |
| **Redesign** | Models, problems, & metrics | To-be models | You (and your team) ideate ways the process could be changed to mitigate issues and/or improve on the ID'd metrics |
| **Implementation** | To-be models | New process executed | This is doing the change. This involves organizational change management, automations, and any other supporting changes |
| **Monitoring** | New processes & metrics | Metrics data & unseen issues | Once the process has changed, you monitor the [[Lagging Measures]] to see if it's had the kind of results you anticipated. |
